چکیده
Background The opioid crisis has illuminated the risks of use for pain management, with renewed interest in reducing consumption after common orthopedic procedures. Anti-inflammatory medication is an important component multimodal management patients undergoing surgery. purpose this study was to evaluate effect celecoxib on control and shoulder Methods Patients scheduled either total replacement (group 1) or rotator cuff repair 2) were candidates study. exclusion criteria included allergy celecoxib, coagulopathy, anticoagulants, baseline long-acting opioids, a history medical conditions such as myocardial infarction stroke. Consenting randomized by type procedure using block randomization receive placebo 1 hour prior 3 weeks postoperatively. primary outcome measure assessed utilization measured morphine-equivalent dose (MED). Secondary measures scores at 6 Data analyzed multiple linear regression. Results Of 1081 from February 2014 2018, 78 enrolled arthroplasty 1, 39 receiving placebo) 79 2, 40 placebo). Compared arm, prescribed took fewer MEDs –168 (95% confidence interval [CI], –272 –64; P < .01) population –197.7 CI, –358 –38; = .02) group. Similarly, weeks, used –199 –356 –42; –270 –524 –16; .04) No statistically significant differences found between arms group, weeks. note, preoperative associated higher levels group (P .01 all) but not 2 > .05 both). Conclusions Use morphine equivalents significantly less who surgery Multimodal effective way reduce postoperative patients. Preoperative arthroplasty.

Preoperative opioid use associated with worse outcomes after anatomic shoulder arthroplasty.
BACKGROUND Preoperative opioid use has been associated with worse clinical outcomes after orthopedic surgery. The purpose of this study was to evaluate the impact of preoperative opioid use on outcomes and patient satisfaction after anatomic total shoulder arthroplasty (TSA). METHODS We identified 224 TSAs performed for primary glenohumeral joint osteoarthritis with 2- to 5-year follow-up in ...
متن کاملGood function after shoulder arthroplasty
BACKGROUND AND PURPOSE Different results after shoulder arthroplasty have been found for different diagnostic groups. We evaluated function, pain, and quality of life after shoulder arthroplasty in 4 diagnostic groups. PATIENTS AND METHODS Patients with shoulder arthroplasties registered in the Norwegian Arthroplasty Register from 1994 through 2008 were posted a questionnaire in 2010. 1,107 p...
متن کاملGelatin matrix use reduces postoperative bleeding after total knee arthroplasty.
Bleeding after total knee arthroplasty can result in significant morbidity and increases the need for blood transfusion. The proper use of intraoperative adjunctive topical hemostatic agents can enhance hemostasis perioperatively, potentially reducing blood transfusions. In this prospective study, 157 consecutive patients undergoing primary total knee arthroplasty received FLOSEAL (FLOSEAL Hemo...
متن کاملUse of preoperative gabapentin significantly reduces postoperative opioid consumption: a meta-analysis
OBJECTIVES Effective postoperative pain management is crucial in the care of surgical patients. Opioids, which are commonly used in managing postoperative pain, have a potential for tolerance and addiction, along with sedating side effects. Gabapentin's use as a multimodal analgesic regimen to treat neuropathic pain has been documented as having favorable side effects. This meta-analysis examin...
متن کاملPreoperative pregabalin administration significantly reduces postoperative opioid consumption and mechanical hyperalgesia after transperitoneal nephrectomy.
BACKGROUND Preoperative administration of pregabalin is proposed as a promising way of enhancing postoperative pain control. Whereas a few studies have investigated the effect of pregabalin on postoperative opioid consumption, no study has focused on the influence on postoperative hyperalgesia. In this randomized, triple-blinded, placebo-controlled study, we aimed to demonstrate that a single, ...
